Overview

All digital forms accounts come with four generic template forms that can be accessed from the Forms page. A library of 25 industry specific forms can be added to your Simpro Digital Forms account for free upon request. These forms can be edited and duplicated as required to meet your business needs. Default forms PDFs are made in an advanced designer. Editing these PDFs using the designer may cause some formatting issues. If you would like to try to change the PDF please make a copy of the original form.

Certificate of Compliance NSW	Certifies that conditions of consent have been satisfied, that work has been properly carried out or that a plan or design complies with a standard.	Electrical	AU - NSW
Fire Safety Statement NSW	A fire safety statement is a document issued by or on behalf of the owner(s) of an existing building. The statement confirms that an accredited practitioner (fire safety) has assessed, inspected and verified the performance of each fire safety measure that applies to the building.	Fire	AU - NSW
Electrical Certificate of Testing	For electrical installations, this certifies that the electrical installation, to the extent it is affected by the electrical work, has been tested to ensure that it is electrically safe and is in accordance with the requirements of the wiring rules. QLD Government - Form 9/RPZ or Backflow Prevention Device Test	This form is used to test RPZ's (Backflow Prevention Devices) once every 12 months (more often if required by the site). Filled out on the customers behalf and sent straight to the local council. A special permit is required to do this testing.	Plumbing	AU - QLD
Variation Sign Off	An approval form for technicians to get approval from site managers or customers before carrying out variations.	All	NZ
Form 12A: Certificate of compliance with inspection, maintenance and reporting procedures	This form is used as proof that the inspection and maintenance procedures listed in the compliance schedule have been carried out. A 12A is completed for each system e.g. HVAC, Fire safety etc., before issuing a Form 12 Warrant of Fitness.	All	NZ
Incident Investigation and Report SSSP Form 9	The Incident Investigation and Report SSSP Form 9 is used as part of the SafePlus program.	All	NZ
Form 12: Warrant of fitness	This form is a written statement issued annually to the council. A copy is also publicly displayed in your building. Issued once all 12As have been completed.	All	NZ
CP12 Gas Safety Certificate	A report which outlines the appliances tested in an annual gas safety check.	Gas and Plumbing	NZ
Electrical Certificate of Compliance & Electrical Safety Certificate	The Electrical Certificate of Compliance & Electrical Safety Certificate tells the customer, customer, electrician or business that the electrical work undertaken complies with the appropriate federal and state safety standards. Producer Statement Construction (PS1), (PS2), (PS2) and (PS4)	A producer statement is a professional opinion based on sound judgement and specialist expertise. It is not a product warranty or guarantee of compliance.		NZ
Electrical Installation Certificate	The Electrical Installation Certificate is a safety Certificate that is issued by a qualified electrician as confirmation that any electrical installation project has been that complies with the BS 7671 (IEE Wiring Regulations)	Electrical	UK
Electrical Installation Condition Report - (EIR Cert)	The Electrical Installation Condition Report - (EIR Cert) is used in reporting on the condition of an existing electrical installation. Reporting is based on the process called Periodic Inspection and Testing. It must be done on regular basis in accordance with BS 7671.	Electrical	UK
Minor Electrical Installation Works Certificate	Confirms that any minor electrical work that has been carried out has been subject to the proper design and construction and has been tested according to the 1IEE Wiring Regulation BS 767s	Electrical	UK
Catering Appliance Gas Safety Certificates (CP42)	A Commercial Kitchen Gas Safety Certificate (CP42) is an official document that proves the gas equipment within a kitchen has been installed safely and correctly.	Gas and Plumbing	UK
Commercial Gas Testing and Purging Forms (CP6)	The Gas Testing and Purging form allows the gas operative to provide a report to their client on the safety of the gas installation, when carrying out strength testing, tightness testing and/or purging in accordance with IGE/UP/1 (Edition 2) and/or IGE/UP/1A (Edition 2) in a non-domestic environment. Domestic Unvented Hot Water Storage Vessel Commissioning/ Inspection Record (CP8)	The Domestic Unvented Hot Water Storage Vessel Commissioning/ Inspection Record (CP8) is used in installing, commissioning or inspecting domestic unvented hot water storage vessels. Gas Installation Safety Report (Non-Domestic) CP17	The Gas Installation Safety Report (Non-Domestic) CP17 is a document that verifies the safety of gas installations in non-domestic premises, such as commercial or industrial buildings. Plant Commissioning/Servicing Record (Non-Domestic) CP15	This form allows gas operatives to accurately record checks carried out while commissioning/servicing non-domestic appliances. Concise details of combustion checks can be recorded plus many more essential tests.	Gas and Plumbing	UK
